I highly value the confidentiality of your personal information.
For that reason I am committed to treating your privacy in accordance with GDPR regulations (25th May 2018), which are legal protection for your personal information. This tells you what personal information I gather and why, and what your rights are - you can find out more on https://www.eugdpr.org.
Information I Collect
Personal information such as your name, contact details, date of birth and relevant medical history, to ensure the appropriate treatment is received.
Who I Share Information With
Sharing your personal information will only be by explicit consent if lawfully required or for referral purposes only. All communications via email and or texts will be deleted in a timely manner, no payment details will be kept and all information will be kept in a locked cabinet.
How Long Will I Store Your Information
I will store your information for 7 years after the date of your treatment, or for 7 years after you have reached the age of 18 if you are/were under 18 when your treatment took place. My lawful basis for processing this personal information is ‘contract’, this means that I have a legal reason to ask you for your data because I need it for contractual reasons and to comply with my Insurance Company.
You do not have to share your personal information with me, but if you do not I cannot offer you a treatment. You must fulfil your side of the contract (share your personal information) in order for me to fulfil mine (carry out treatment).
Accessing Your Information
You have a right to access your personal information within one month of request. If you are unhappy about how your information is collected and stored, you have the right to complain to the Information Commissioners Office (ICO), with whom I am registered - https://ico.org.uk/
